For what it's worth, I tried installing Irfanview (32bits) and associated the CMYK file (attached, zipped to avoid any site compatibility issues) with that. You can check whether it's really CMYK.
The desktop thumbnail, file explorer and preview pane still showed it correctly.

Are you using the 64 bit version?

I am not sure if the color profile (ICC) has anything to do with the thumbnails. It must be some bug with the associated application producing the thumbnails. Since the image is CMYK and the thumbnail is RGB, the application should use the negative values. This means it should produce 255-C red, 255-Y blue and 255-M green. Instead it mistakes the image for RGB and produces a negative image by displaying R=C, G=Y and B=M. I have also seen some weird thumbnails such as BMP or PNG files having a black background instead of white. Try associating the files with another application and see if there is any difference. You may have to restart the computer for the changes to take effect.

Can you upload one of the images in question?
The image has 3508 x 3071 px and 2.97 MB.

Before sending it here and to test an intuition, I made a copy of it in Windows Explorer via drag and drop. The copy was displayed in negative, just like the original.

I opened the copy in IrfanView and resized it to 800 x 700 px (proportions preserved). The result was that they appear differently in Explorer:

The inevitable question now is: size matters?

I did it before creating the thread, forgot to mention. But if it may help, a few weeks ago I applied this fix here:

Well I've never applied that fix. Personally I keep thumbnails disabled in Explorer and use other third party apps for browsing images. Just for consideration maybe read this:

After Creators Update 2 thumbnails cache keeps auto-deleting Solved - Page 12 - Windows 10 Forums

I do remember having a similar problem but it was too long ago to remember how I fixed it. I might have uninstalled all photo software completely along with shell extensions but that's just a guess.
