You don't need the latest driver. Go back to 417.71
Nor do you need DCH drivers which will only complicate things even more.
I have been using Nvidia Graphics Cards for decades and never install the latest as they are effectively Beta drivers, which includes DCH, some disappear from the Nvidia list unsurprisingly.
Once again, this has nothing to do with your cables, converter or your KVM switch.
This is not a mixture. It's clearly just a driver issue which the others described well. Just follow what they said and you should be fine.

I re-installed a clean Windows and the latest version was successfully installed.
But the VGA cable (or that mixture) has reduced my monitor's quality a little bit.
