Custom Tiles, no really, custom "tiles" see pic.
I didn't find a post about this, only about the number and layout of the tiles so I'm sharing what I found.

Pretty easy, you go find some box art or other art you like for your game/app and save it to a folder you've made to store this stuff should you need to redo it later and browse to it in the program in the 150x150 box, after selecting the app in the list (you can manually browse to app not in your start menu too) , hit save and you have custom small and medium tiles that look awesome. (Author says other sizes to be added eventually)
I was confused at first, you only need to select an application target and an image for the 150x150, then hit save. The new tile image will get named automatically and saved in the applications folder, unless you input a custom name before hitting save. You do not need to enter anything into the 75x75 boxes.
