1st you did a great job giving all the data available in your zip file. Nice.

So even though other mention other things to try look at the motherboard, memory and fans to insure they are working properly.
The key here is you said it has happened on even after you reinstalled Windows
so my guess is hardware or driver issues.
I do see the video driver issues in the dump.

    My off the wall suggestions.....

Open your PC desktop and with power unplugged unplug and reinstall the memory modules.
Also if your fans are dirty clean them carefully and remove all built up dust on the blades.
Also insure your CPU fan is working constantly.
If your CPU fan is not running at its max speed you may be getting the CPU overheating and shutting down.

Place the computer in safe mode: (shift + restart)
Boot into Safe Mode on Windows 10

    a) repeat HD Tune benchmark while in safe mode

    2) Update the thread with HD Tune full error scan when available

    3) The results display the model and that can be used on Amazon, etc. to view the product (nothing to hide)

    4) Run HD Sentinel: (free or trial edition)
Hard Disk Sentinel - HDD health and temperature monitoring

Post images of each of these tabs into the thread:
