On Win10 1803 on an old laptop. When I go to Windows Update, it shows that I'm up to date, and also that 1803 is end of life. So I click on Update and it automatically starts downloading 1903. After download is complete, I click on "Restart Now".
After restart, I'm still on 1803. Rinse and repeat the above 2x.
What am I doing wrong?
That doesn't sound right. After it downloads there is quite a bit of processing and it shows this Install process as percentages complete.
Once installed it asks for a restart which allows Windows to change the files the current installation had locked. Once restarted there is a blue screen and Windows reports on its completion progress.
Interesting that it is offering you 1903, currently it should be either offering 1909 or V2004 (assuming it thinks the install will be successful).
I would run the Reset Windows update to flush what is present.

After this is run could you also do a
sfc /scannow (Use a Command Prompt(admin) or Powershell(admin) prompt, right click Start, command is in list.)
See if updates is now offering you V1909. Start > Settings > Updates & Security > Check for updates
As of 1809 it does not auto install. Given you are back on 1803 it may just kick the process off.
Your machine should have been forced to update earlier. Are you using any special privacy / blocking tools.
One thing that definitely interferes with Feature updates is third party AV's. If you have installed you must uninstall, not disable.

Thanks for pointing out potential AV issues. The family member using the laptop had installed Norton 360, so I uninstalled that, then restarted. I ran the batch file to reset Win Update. After restart, I went back to Update and as before, it says I'm up to date. I click on Check for Updates and it's downloading 1903 again. Maybe this time it will work. Will report back.

Because 1803 is so old lots cant update from it but if you create a image of the system then do a clean installl it seems to work just fine. They made changes to the setup and the 1803 doesnt have the up to date setting for the newer setup
OK, so after uninstalling Norton 360 and resetting Windows Update, when I run Update, it downloads 1903, and then in Update, it shows installation progress. After installation progress completed, it prompts me to restart. And then it shows me blue screen and Configuring update . . . but the progress is going way too quickly to actually be updating the OS.
And sure enough, it's still on 1803.

OK, so even if I download an ISO and make a bootable Win10 USB, I won't be able to update the OS?
If that's unlikely to work, I'll have my relative backup all her data and do a clean install.
I've never hears that, that is old version can't be updated.
Media for version 2004 is available with MCT tool. I would image machine, then download iso to desktop using media creation tool. Right click ISO, mount and double click setup.

Once tool is downloaded execute. Follow prompts to get ISO and save to desktop. Then as I said above right click, mount, run setup.
Imaging is an important step as it provides you with what should be a proven restore point.
I like how something barely two years old is now "so old". And so old that it causes problems when you try to update it. How long did they go in between full OS changes in the past? Just because they've put out three or four "changes" in 2 years (they are not by anyone's definition really even "new") shouldn't mean people have so much trouble if they missed a few updates.
