Sorry for the delay, but I've been busy.

I have SOLVED this - and this may not be the fix for everyone, but it fixed 600 machines I own.

The App Readiness Service was disabled, once it was set to Automatic again BANG - all machines took the updates.
Thank you so much! This fixed my problem, I had App Readiness Service disabled. Once re-enabled, it worked fine.

Another potential cause of this problem: reagentc /enable failing with error code 3bc3, Windows Update failing with error 800F0922 - Super User

If you ever had some problem with the EFI partition, this will likely work for you.

In summary, you should check the ID of the EFI partition, and if it's ebd0a0a2-b9e5-4433-87c0-68b6b72699c7, you should set it to c12a7328-f81f-11d2-ba4b-00a0c93ec93b. Doing so fixed a lot of weird problems in my system.
