If the update was KB4457128 then it has been observed that it can prematurely record a successful install before a restart has taken place...

Manually restarting would seem to complete the install, but with 'success' recorded twice in the update history.

You said 'all our computers', does that mean you are in a company system and are using Windows Server Update Services (WSUS) to manage and distribute updates? If so, try getting a new copy of KB4457128 as Microsoft appear to have made some changes, at least for the public update servers...

Having ended my testing I restored my test machine's 17134.228 image and updated that to KB4457128, I have also clean-installed 1803 on another machine then updated it. If you are not using WSUS, as KB4464218 contains no new security fixes it will not be downloaded by the automatic windows update check. It will however be found and installed by a manual check. Open the settings for Windows Update and click 'Check for updates' to find and install it.
