    Welcome to TenForums @acount300

If the update was KB4457128 then it has been observed that it can prematurely record a successful install before a restart has taken place...
    Cumulative Update KB4457128 Windows 10 v1803 Build 17134.285 Sept. 11

    ...and...
...It would appear that the prematurely logged 'success' also prevented a restart being scheduled...
What Times WU To Auto Check For Updates?

Manually restarting would seem to complete the install, but with 'success' recorded twice in the update history.

You said 'all our computers', does that mean you are in a company system and are using Windows Server Update Services (WSUS) to manage and distribute updates? If so, try getting a new copy of KB4457128 as Microsoft appear to have made some changes, at least for the public update servers...

This all seems to have changed since about the 14th though. Having ended my testing I restored my test machine's 17134.228 image and updated that to KB4457128, I have also clean-installed 1803 on another machine then updated it. MS seem to have changed something, as KB4457128 now records a successful install just the once on both those machines.

Or you may want to get the new KB4464218 that was issued after KB4457128.
    Cumulative Update KB4464218 Windows 10 v1803 Build 17134.286 Sept. 17

If you are not using WSUS, as KB4464218 contains no new security fixes it will not be downloaded by the automatic windows update check. It will however be found and installed by a manual check. Open the settings for Windows Update and click 'Check for updates' to find and install it.

    Hey acount300,
I think what you're referring to is located in ... Settings > Update & Security > Windows Update > Advanced Options > Well show a reminder when we're going to restart ... bla, bla, bla
