Well, my attempt to install 16215 using Kari's workaround instructions was unsuccessful, crashed with INACCESSIBLE BOOT DEVICE after the prolonged, "installing" then booting once and beginning the first stage of the real update, Last % is saw was 10, IIRC. About the same place as my attempt to upgrade from the ISO. Maybe a faulty ISO?

Now I see your new suggestion, Kari. Will look at that tomorrow
I just had this problem. Mine was because windows is ACHI and somehow the Bios had reset the drives back to IDE. so it would crash with that inaccessible boot device half way through booting.

I think a new build will be released this week, but of course we can never be sure until it really happens.
    Kari,

I see that build 16226 has been released however my Windows Update is still trying to install 16215 unsuccessfully and with the same error code!

How do I bypass 16215 and get the download for 16226?

Yes I will make a new bootable iso before I restart this time!

    Thanks,

If an upgrade fails, when you are back on desktop run an extended disk clean-up (tutorial) selecting everything there is to delete. When done, open Settings app > Update & Security, the Restart now button should still be there although all upgrade files have been deleted.

Click the button, Windows restarts and tries to upgrade which of course fails once again as upgrade files are missing. This won't take long, when back on desktop check for updates again, Windows Update should now find 16226.

Here is what happens when i paste your Command at an elevated prompt using Powershell:

    C:\WINDOWS\system32> %SystemRoot%\System32\Cmd.exe /c Cleanmgr /sageset:65535 "&" Cleanmgr /sagerun:65535
%SystemRoot%\System32\Cmd.exe : The module '%SystemRoot%' could not be loaded. For more information, run
    'Import-Module %SystemRoot%'.
    At line:1 char:1
    + %SystemRoot%\System32\Cmd.exe /c Cleanmgr /sageset:65535 "&" Cleanmgr ...
    + ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
    + CategoryInfo : ObjectNotFound: (%SystemRoot%\System32\Cmd.exe:String) [], CommandNotFoundException
    + FullyQualifiedErrorId : CouldNotAutoLoadModule

i swear the rest of you will be on to build number 25036 (or close) and i will still be updating and failing 16215!!!

Do you see there any mention of PowerShell?

An elevated PowerShell is not the same as an elevated Command Prompt. When following advice given, it is recommended to do exatcly as told in instructions.

Had this FIRST_BOOT error plaguing my PC, tried everything purposed in this thread but nothing helped.
Eventually solved the problem for my specific case:
If you own a Samsung SSD, and your Windows is installed on it, install Samsung Magician (if not already installed), and make sure you turn off "RAPID Mode" in Magician before attempting to update your Windows. You can re-enable RAPID Mode after updating is done.

    This solved my issue instantly.
Thanks I will try that the next update, but 16226 updated fine with it on. But other builds did give me issues.

I left it off (RAPID mode isn't recommended for normal every-day usage), and also updated from 16215 to 16226 smoothly.
I suspect that FIRST_BOOT issues are usually related to the hard-drive, be it special modes or IDE mode instead of AHCI.
I'd advice to check these out before trying anything else.

I use AHCI and I have used Rapid Mode every day for 3 years without any issues. Samsung never said it was not for every day use. Some say that Win 10 does not even need Magician but others say it does. I find my machine to be faster with irt on, but turning it off during an upgrade may be a good idea. On one update I had to manually shut it down before reboot.

Samsung wouldn't say that, but look for benchmarks, the RAPID mode dropped performance quite significantly.
I personally don't see any difference so I keep it off.

    Hi,

What benchmarking software would that be ?
Personally I have it on and do notice the difference. All depends on what you do with the machine, I suppose.

    Cheers,
