I also tried to create an ISO, which worked. Then running setup.exe is after mounting it worked, looked good, it was preparing and doing all the little checks with progress indicator etc, then the ModernUpdate.exe or whatever it's called crashed and the whole thing stopped. Bah!

Ok I followed every single step in this list, but unfortunately to no avail.

Disabled every single service except for the Microsoft and Intel ones.
Ran all the disk checks which all reported zero errors.
Also previously uninstalled SQL Server just because someone guys online said that may cause issues.

This build must be garbage, my system is very clean and it's a brand new Surface Pro 2017 with 1Tb disk.
Based on your reply, you did not follow my instructions to the letter. When I said disable all non-MS services, then I mean ALL non-MS services.

Are you absolutely sure you ran ALL steps? Are you absolutely certain any AV was completely disabled? What messages did SFC /SCANNOW command give you when it completed?

What error messages did you get during upgrade?

It is really difficult to help if we don't get detailed information about what you have done, what Windows did and what messages it gave you.

    Install went smooth here..

They fixed the issue with MSI Afterburner/Adia64 and other hardware monitoring programs.

According to your post (as I read and understand it), you restarted before making ISO and upgrade then failed. If you do not make ISO before clicking Restart now, and upgrade then fails, the UUP folder is no longer intact and the UUPtoISO process most often fails.

I have repeatedly reminded you geeks that you should do the ISO before clicking Restart now, and only if the button really says Restart now. If it says Install, you need first click it and let WU install the update (read: create UUP folder content). When done, the button changes to Restart now, at which point you should make the ISO.

Yes true, but I did not in fact press restart anyway, as it was so slow installing, I had gone to bed, that it decided to automatically upgrade without me pressing restart as it was outside active hours!

However, I have not given up yet - I have copied files from uuptoiso folder to an iso using imgbrn to see if that will work. If not I will restore 16281 (the new share vm method is great) and this time watch it during active hours.

So the only tiny variation to your steps that I did was to leave the 3 Intel services running which come with the surface pro (they are just the surface pro drivers etc), not any other junk that's been installed later etc.

I don't run antivirus software so it's not that.

The SFC /SCANNOW reported something like "No errors found" or "No issues found" and it fully got to 100% complete and then gracefully ended. It found absolutely nothing.

During the upgrade, my machine reboots a couple of times and I see a few things like "We're preparing your updates..." (with spinning progress indicator etc). Then not long after that, the screen blinks and the larger progress indicator comes up showing "Restoring your previous version of Windows" and then within like 10 seconds I'm back in Windows running the same old build 16281 again.

In Windows Update install history, I have the following error numbers:
0x80240034 (heaps of these, but I think that was a while ago now and maybe that was the issue MS fixed)
    0x80070002 (8 of these)
    0x80070003 (2 of these)

This is also something I have mentioned a few times in various threads: Why don't you adjust active hours according to your needs?

WU allows an 18 hours long active hours period. Personally, I wouldn't even think of going to bed when upgrade is not ready for ISO making without first adjusting active hours, as I did last night. When going to bed after 1:30 AM when Skippy build 16362 was still installing. I simply switched active hours to last until noon today, which allowed me to go to bed without stress and make ISO early this morning without any whatsoever issues.

Only then did I restart and let WU upgrade.

Disable ALL non-Windows services, and DISABLE Windows Defender AV completely. You might not be aware of this but Windows Defender is on by default and causes a lot of upgrade issues for many. Disabling any AV can also improve upgrade times by 4-20x depending on your system.

Your errors tells me that your issue is definitely caused by Windows Defender. Especially codes 0x80070002 and 0x80070003 that means some files are missing.

Try again, now following my guide to the letter.

Ok let's see, I've now disabled everything in Windows Defender and am getting the "Enable virus scanning software!" warnings from Windows because I did that. Now I'm going back up to run through all your other steps in full.

Now silly question: Why would they release builds that Windows Defender interferes with?!?!? I mean, they wrote both pieces of software.. Is this a known UUP issue?

Everything as it should be here. Slow old system but with SSD and evn second time it finished in less than an hour. Managed to catch UUP to ISO in time and all is hanky-dory. Now to clean up the mess with disk management. Figured it's easiest using MR and omitting unnecessary partitions.
