Oddly, the 1909 ISO says it contains 18362.418, not 18363.

This seems to be correct, as an earlier Insider ISO from UUPDump said the same thing but still installed 1909 build 18363. The step up from 18362 to 18363 is made by an 'enablement package' which seems to be included in these 1909 ISOs.

I haven't tried installing this ISO, but when I tried the UUPDump 1909 ISO a week or so ago it installed the 18363.385 build that was on the ISO and, after installing, Windows Update got the latest CU.

1909 and 1903 share the same updates, so I expect the official 1909 ISO to install 18363.418 which will immediately update to today's 'Patch Tuesday' CU.

Is there meant to be a big update today or something?

My current build is 18363.449, as I've previously installed the kb4517245. Do i need to have this removed before installing the latest ISO?

It's 'Patch Tuesday', the second Tuesday of the month. There's always a cumulative update with the latest security fixes issued on Patch Tuesday. It is also highly likely the Feature Update to version 1909 is going to be released as well.

No, you have already got 1909, so you'll just get the latest CU today like everyone else. These still on 1903 are likely to see the Feature Update to 1909 as an optional extra.
