I also wonder why all the CU's (for myself and some others) fail to install first time around. That happened again today and so I restored back and reapplied via the update catalogue.
I am exactly in the same boat. For some weird reasons, the last 4 or five times I had tried to install a CU on my main machine, it would also fail the first time around with the infamous message at reboot: " We could not install the update. Undoing changes. Please don't turn off your computer". Right after, I troubleshooted WU and no problem was found. I ran dism….restorehealth, reset WU with the help of Brink's tutorial, downloaded the standalone MSU from the MS Update Catalog to finally get it (KB4469342) installed. Everything went smooth though on my second machine which is way older.

It failed because a Servicing Stack Update was needed first. For some reason Servicing Stack Updates never show in the history. This also happened in the Release Preview when .168 and earlier releases went in.
Glad to know that. Is there a way to find out which service stack is already installed, since it doesn't show in the update history?. Furthermore, if a service stack is missing, ain't MS supposed to install it first before the CU?

I don't think there is a way to tell. For me I don't even notice the failure because it fails and automatically retries before the restart.

Applying the SS update before the CU is a bit too sensible for MS.

I think it is, but on the other hand, I don't know why I would be missing a SS Update since I have always installed all MS's updates religiously.

    Hello Tony,

Double check to see if the Servicing Stack KB4470788 below has already been installed first. It's required to get KB4469342.

not seeing that one in installed updates under programs and features or in other updates in the windows update section at all. unless i missing it some how but i don't see it, should i go fetch it, but if it is there and i'm missing it would it simply just install over it or not install.
