You could wait, but I haven't had any issues with 1809 on my systems...
I upgraded four machines to 1809 back before the release was paused. Three are still running 1809 (now update to 17763.134) without any problems. but on the one machine I use for Hyper-V virtual machines I had an obscure networking issue (which I reported on 8th. October) so imaged it back to 1803. I have just tried upgrading with the re-released ISO and it still has the same issue.

That was a networking problem in its Hyper-V WiFi network switch. While the host machine worked perfectly on the Internet and with two-way communication with the VMs, the host machine couldn't write to shares on other physical machines on the network (but it could read from them perfectly). With Hyper-V removed it had no problems though.

I would recommend to create an image backup first to be extra safe though
For now, that one machine has been re-imaged back to 1803 (again) and is staying that way until this issue gets fixed. The others are staying on 1809.

Is v1809 being distributed through Windows Update at this time (11/17/18), or do people have to install it manually?

It doesn't seem to be rolling out through windows update yet, so manually would be the way if you want it straight away.

I have two machines on 1803. Clicking 'check for updates' does not find the 'Feature update to Windows 10, version 1809'. I have left my test machine running 1803 non-stop for the past three days, so far the daily automatic check hasn't found the 1809 update either.

I used this link to update my laptop (turtletop), and my newer Acer desktop. I used the creation tool to do a clean installation on my Dell desktop. My computers are all running quite nicely now.

Like Bree, I have my system sitting at v.1803 waiting for WU to auto-update to v.1809. It checks every day, but no big update. However, yesterday it did auto-update with KB4023057. I had left the system on all night as Friday night is when NIS runs a full system scan, which takes several hours and consumes a fair amount of resources, and sometime during the defined idle time, WU did the update. Perhaps this is a prelude for v.1809?

"This update includes reliability improvements to Windows Update Service components in Windows 10, versions 1507, 1511, 1607, 1703, 1709, and 1803. It may also take steps to free up disk space on your device if you do not have enough disk space to install Windows updates."

    Yes, mine got that too.

This appears to be a 'helper app' designed to help free up space on small drives as and when 1809 arrives. It does not of itself seem to trigger the features update, rather it will lie dormant until windows update delivers 1809. KB4023057 seems to get re-issued in advance of each Features Update. Here's the one prior to 1803.

When a Windows feature update is available for your device, you may see a message on the Windows Update settings page or elsewhere that asks you to free up disk space by removing files or applications that you do not use regularly. If you see this message, select Fix issues to free up disk space on your device.

    Always!
I actually created a Macrium image back in October before 1809 was first released and then halted. That would take me back to 17134.286 which wouldn't be a disaster as I'm only on 17134.345 now. Plus I keep a note of any serious S/W updates that occur between images. On my main PC, only VLC has had such an update.

If you want an iso use the MS media creation tool and let it bring you the current iso.
I'm wanting to give Enterprise 2019 LTSB a try to see if its usable as my daily driver. You can't install that via the MCT ISO. It was removed from My Visual Studio subscription when 1809 was pulled.
Plus I'd like to have the Visual Studio 1809 Consumer and Business ISO's. The MCT ISO is the Consumer ISO. I have the MCT ISO and will use it if need be.
