Here is a problem report for the KB4100347 on Windows 10 1803.
After updating the DPC latency went through the roof while accessing USB flash through external (PCI-USB) card. The mouse is jerky, the sound freezes, etc.
System interrupts takes up to 20% CPU in Task Manager.
LatencyMon shows up to 500 ms DPC execution time for USBPORT.SYS
So the PCI USB card is effectively unusable.
The MB USB ports are fine.
Do not have any other PCI/PCI-E controllers to test.

My hardware is CPU: i5-2500K, Chipset: Z68, NEC PCI to USB 2.0 board (VEN: 1033 DEV: 0035) using MS drivers (USBPORT.SYS) ver. 10.0.17134.1

Kinda have no hopes for this to be fixed, cause Microsoft would blame Intel and the opposite.
But does anyone have any ideas of a best place/places to get this reported?

Uninstall first to make sure update is the culprit.

Suggest a USB header for motherboard or external USB hub, nothing but issues with 3rd party USB controllers.

Been using it for don't know how many years without any problems. And the problem is not with the board (would buy a USB 3.0 board, or find some USB headers), but the fact that this microcode update messes up a lot. No idea what happens here, but probably has something to do with DMA.

Any way of disabling the new Spectre fix? I've tried with the InSpectre app but it doesn't seem to work... is a reboot needed?

You didn't mention what kind of problem(s) you are having for wanting to disable the new Spectre fix. At any rate, whenever a modification is made in the registry, it is always a good next step to reboot your machine in order for it to take effect.

After reboot, if you are still having the same issue (performance hit probably), why not uninstall KB4100347 and put Windows Update to pause in settings.

If you've done the removal of this update to verify the issue has gone, fair enough. If you're saying that it's misbehaved since this update and haven't done the removal, then you don't believe in something called regressive testing. It's key to properly diagnosing problems. Not system restore or re-imaging, removal of the specific change you are saying caused the issue.

    Is that regressive enough?

The real question is: Does anyone have any ideas of a best place/places to get this reported?
Cause the Microsoft support forums are next to useless, no MS staff there, just users helping other users.

Why am I getting this on an 8 year-old Lenovo Thinkpad (Clarksfield, CPUID 106E5) when it is not in the list of processors to update and I was under the understanding that anything this old would not get anything?

Because they don't check anything. They just push it.
If it installs you're ok, if not no harm done!
