    I wish people would stop using "Bug" to describe an issue - A bug is something that causes a program break on every system it's installed on
    This really is not the case. A bug is simply when any code does not do its intended job in all situations.

    It is not a prerequisite of a bug that it affects every body.

    People use terms like issue as it suggests a deeper meaning rather than an outright admission the coding is screwed up.

    As an aside, most people are not even aware of the derivation of the term bug. It dates back to early days of computing when an early computer with manual relays.

    From wikipedia:-
    The term "bug" was used in an account by computer pioneer Grace Hopper, who publicized the cause of a malfunction in an early electromechanical computer. ... Operators traced an error in the Mark II to a moth trapped in a relay, coining the term bug. This bug was carefully removed and taped to the log book.

    This is how I understand it too and here are a few definitions if we start to dissect this into definition fragments:

    BUG = FAULT
    FAULT = A manifestation of an error in software. A fault, if encountered may cause a failure.
    ERROR = A human action that produces an incorrect result.
    FAILURE = Deviation of the software from its expected delivery or service.

    Isn't this what is happening when some users press the button and they experience a FAULT that produces an ERROR and causes an application FAILURE? (User press a button and the whole application crash instead of bringing up the correct dialogue) :)

    Not sure I see how a FAULT can cause a "human" ERROR by your definitions . An ERROR could lead to a FAULT.
