Hello, yes it appears this is a side effect of the latest version of Windows 10.
On 1903 when you disable SecurityHealthService smartscreen.exe does not run in the background at all. I can attest this from using Windows 10 1903 for 2 years.
All you need to do is make a fresh install of 1903 and Specify_target_Feature_Update_version_to_1903 is the only real way to stop Windows 10 from auto installing newer versions of Windows 10.
I had also problems with 1909 where you can't disable Windows Defender. The security settings revert back to default settings, making it impossible to disable it. And this is part of the same tamper protection for the smartscreen as well. If you able to disable antimalware service executable from running in the background, this will indicate that the rest is also possible. But if you see it is continuing to run in the background even after adding the settings then that explains why you have problems with these smartscreen settings reverting back.

Anyway to roll back a machine without a fresh install? Any way to just uninstall the updates?
The reason I ask: I have a total of 5 machines with the problem which each have a load of configured apps. I can't imagine the task of wiping out each machine and starting it from scratch.
