Why does Cortana respond to TV conversations?
For example, I had the TV going, someone said "Thank you" (not preceded with Hey Cortana), Cortana opened up and said "No problem". This happens all the time.

Maybe because Cortana can hear them? Or maybe because there's built-in phrases that are recognized? Maybe the microphone is aimed at or too close to speakers? Maybe the mic is not noise-cancelling, picks up background noise in the room? I don't have a need for Cortana so those are merely guesses. Why does Cortana respond to TV conversations?
No doubt the microphone is picking up the TV dialogue, but the point is no one on TV is saying the wake phrase, "Hey Cortana".

Cortana has changed a lot from 1903 to 20H2.
In 20H2 there seems to be nothing in Settings for Cortana = no Hey Cortana option. (Searching for Cortana -> no results).
In the app as it now is, there's a Settings option, and it refers to the 'wake' word as 'Cortana'.
Now just how confusing can MS get?
Actually that takes you to this in Settings:
