Making a Form on phone delivering to a computer

So this is a wierd one, i work for the local goverment in my city in Norway.

We have a ... "Norwegian" issue during the winter... Snow, tons and tons of snow.

So what im trying to do is to make a form for complaining about snow clearing or where its needed for the people we have working outside for the local goverment.

When the form is filled (on a android or iOs device) it would then be delivered either by mail or maybe uploaded to a cloud server ( or just a server we have) for the leader of road maintenance so he gets a overview.

He asked for this sulution and i have been googling abit but im not sure on how to do it.

So if anyone has an App recomandation that lets me do it or an idea id love some input.

Does your agency have a software development organization? An IT support organization?

If so, discuss your needs with them.
If not, look for an outsource solution.

Take a look at this. You can build a simple app with no programming required, May be good enough for your needs:

The reason I asked is ... there needs to be a operating process. This form you seek will accept complaints. But what is the process for handling the complaints?
To put it another way ... what would be wrong with having people just send you emails? How is that any different from having an online form?

Well its abit akward for me to answer since im not the one asking for it.

But apperently the elderly gents out in the field wants to be able to " Quickly report complains" over the phone. in a pre filled form. that will then be delivered to a manager in any way.

Writing an email takes time on the phone apperently. Their words not mine

Ive made several check boxes on screens and text fields to get filled in ( i have a form im following)

at the last page ive added it so it automaticly sends a file to the email of the manager. But how do i get it to send the results from the previous pages to said file?

Im unsure about how to. " create the file" with their answers to the form.

This is how my city has their report part of the city website set up. I Want to Report... City of Springfield - Home of Abraham Lincoln

Does your city use GPS for tracking the equipment? You could incorporate real time plow reports on a map if not already.

Send an email to my city about who they used to help with updating the website. They should be able to help out with this.

Sorry, I have no idea. I just found the MIT thing via a Google search and have not even played with it yet. Maybe contact the author and pay for his services?
