The only way I could resolve this is with a fresh Windows installation.

The drive was damaged. There was no way to image it. I was able to do a clean install of Windows on the SSD and then copy files from the drive.

I could find no solution to this request. It would make a great addition to the OS to be able to associate a specific data file to a specific executable.

It appears that the drive was corrupted somehow, or configured with a third party utility. I was able to zero the drive and get it working again.

This issue was never resolved, and I no longer need the solution.

The only solution that I found was to use an old Windows XP computer for that software.

Follow up... they decided to just use PINs, so this question is now moot.

I finally found the culprit here. It was the DOCK that the laptop was mounted in. Swapped it for an identical one and the issue is gone. I now have perfect video and audio.
