I did all the above steps, checked cable, adjusted DHCP table, etc.. and still some devices are getting 192.168.0.XXX even after the ISP router/modem is in IP passthrough.

Got it figured out, I called my ISP and they remotely changed their modem/router to bridge mode, so far so good.
