We're trying to hit a moving target here. The clean install screenshot in #22 would have been the one to resolve as that's pure Windows. After that it's games related as the OP immediately additionally installed games software which means I can't now make out what's happening. Maybe someone else can help.

SMART parameters are sometimes rough to interpret. But at least it gives us decimal values and throws the red lines for you to monitor :)
To get a confirmation of severity (likely none if your drive seems ok), I suggest you post a screen shot and ask Kyle for advice here:

Surprisingly after everything has been running for a day after the re-install of a fresh copy... the windows system seems to be running fine. Games, however, still seem to have these random massive lags where the disk rockets to 100% and I hear the same sound over and over and over, until it subsides.
Also, while my friend was looking at it, we noticed this:
Assuming Crystal Disk Info is correct, Immediate backup needed, that's a critical warning, and could well be the cause of the 100% disk usage due to the drive repeatingly trying and failing to reallocate sectors, more than likely to fail at some point.
Well I replaced the HDD and everything is working fine. It hit 100% a few times mid installing a lot of stuff (see below), but the system is CONSIDERABLY faster and more responsive. No freezing/lagging/wtfing... You guys, and a friend, said it was a failing harddrive and it seems as if it was.
