I found similar in some PCs: sometimes the scheduled defrag doesn't run as scheduled, and it's likely because there's something that's interfering with the system that doesn't make it idle. Given that, maybe a third-party program that follows the prefetch layout and algorithm (e.g., don't bother defragmenting large files) can be used.
Also, as an aside, I'm looking at the possibility that real-time CPU optimization might also help, i.e., those that restrain programs that use CPU power unnecessarily. An example is CPUBalance Pro from Process Lasso, which is free. This might also decrease the need for more defragmenting.
