Dear Beardo and Dryden i've the same problem with a clean install for windows 10. The issue always happen when reproducing audio/video files; VLC, youtube, mpc-hc are the apps involved. Recently i'm testing with MPC-HC because with ctrl+j, i can "see" the stutter. For information it happends two times in the last two days after 41minutes in reproducing a video with mpc-hc, first video was a mkv with hw acceleration enable, the second one was an avi with no hw acceleration.
    My HTPC is the following

    Asrock N3700-ITX
    8gb of ram
    Intel HD Graphics
    Realtek Ethernet, and Audio devices.
    Samsung SSD 850 evo
    Panasonic Bluray
    External sasmung disk for p2p

Obv, all my drivers, programs and windows files are updated and not corrupted.

Please advise in case you solve the problem, i will do the same.

Disable updates by changing your network system to one with a metered connection.
    What does this mean?

I'm experiencing the same issues as OP. Explanation is to "disable updates changing network system to one with a metered connection" which doesn't say much to me. What do I do where to fix the spikes?

The entire system is affected regardless if I'm in Windows Explorer or playing a game in 3D, the stutter is like .2 seconds and gives stutter to sound as well, a very weird sound. Like the one in the video one page back or OP youtube clip.

Might try image your hard drive first . Macrium Reflect. Or use another drive. To do a clean install. Create latest iso with Windows media creation. Disconnect all hardware down to basics. mouse, keyboard, 1 drive. Disconnect any odd hardware. Only have display card plugged in. Disconnect all other USB devices. Update to newest motherboard bios or SSD drive firmware. Test hard drive with manufacture test program for example Western Digital Life Guard diagnostics. Do not over clock.

    Now do a clean install.
