Considering the continuous stream of issues you're having with that machine i suggest a ground up install from a legitimate instal source.
No point jumping to conclusions about the hard drive when there is no parameters given that suggest that line of thinking. OP is using a 775 based platform which is extremely slow and nearly 20 years old platform in the wrong hands it is an extremely poor platform.
On the the flip side its very competent still if given the right workloads and used in the right hands it can do some remarkable stuff still.
To the average user its a super slow paper weight and snail of a machine and it does not take much to exacerbate this effect. To someone more involved its a little engine that still can. Emphasis on little engine here though as its pretty little by today's standards.
User proficiency combined with dated platform does not usually end well. Not trying to sound harsh but its the truth some people are more computer literate than others and you need good awareness when using a platform this old.

You still! need to fill out "Computer Specs" so we can give the correct guidance. My dad also has a Dell 755... but it's a quad core (Q6600), 250Gb ssd and I've maxed out the ram. Boots well under 30's no problem.
