Looks good. According to review it is a Realtek radio.
Who knows on the download. If the insides are flaky or are being influenced by noise, who knows.
and if it is card two suspects will have been eliminated. Solid progress on this 18 month mystery.

I got an email notification that you responded to my post whilst downloading the drivers on my phone and looking at them to see who the radio manufacturer was.. low and behold it's the infamous Realtek *sigh*.
Should I cancel the order and find a different wireless card? I tend to be impulsive when buying things I'm excited for and don't get what I really could (meaning I'm not getting the best bang for my buck).
My fingers, toes, legs, arms, and everything in between are crossed hoping it's the wireless card causing all of these issues.
No Realtek makes tons of quality stuff. Likely a poor implementation of their chip caused issue.
This may sound stupid, but no. I can't remember exactly why, but I'm pretty sure my GPU hangs too low and covers one of the two PCI slots that I can actually use. I may be mistaken, but I'm fairly sure that's why. I'm not home right now, so I can't check. Look at this picture: https://www.asrock.com/mb/photo/H97M%20Pro4(L2).png
You can see the the middle two slots (PCI slots) are offset compared to the top and bottom slots (PCIe). My GPU covers the 2nd slot (first PCI slot), so I can't use that one for the wireless card and have to use the 3rd slot (second PCI slot). Moving the GPU to the bottom slot isn't an option because there won't be enough airflow.
Edit: Wait, no... The reason I can't try another slot is because there is only one open PCIe slot. I was thinking the wireless card I have is PCI not PCIe. Ignore pretty much everything I said above.
I keep editing this post because I think of new things to say: My CPU does have onboard graphics, so I might pull the GPU and put the wireless card in that slot just for testing purposes. One thing I noticed while looking for new wireless cards is that there are more USB wireless adapters than PCI or PCIe. Do you have any experience with these? Also, do ethernet wireless adapters exist? Meaning on the card = Ethernet male to be plugged into an ethernet port.
Just got an email from Newegg. The new card has been shipped and estimated delivery is Saturday, so that'd be great.

They use to make ethernet to wifi.
The netgear one I have is selling for hundreds of dollars on Amazon. Wow, technology I bought that went up in price.
Saw some on Amazon. Took power from usb port and looks like you use patch cord between.
My netgear unit had a tiny power brick. with a little jack to plug in. For its purpose it was great as I was using it to make printer wireless and had no usb ports.
The adapter needs power and the ethernet adapter on MB NIC does not supply.
Not many people have a POE router or POE switch.
So a patch cord between dongle and NIC for lan signal and a USB plug for power to energize dongle.
Quite an elegant solution if the device has USB ports.
