First line formatting of Word 365 changes every time it is opened

Hello - I have a Word 365 document that was created from scratch. The first line of this document is "Procedure Name [hanging indent set at 2"] Procedure Name". The entire line is in boldfaced Arial font. The first "Procedure Name" is in 13 pt size and the second is in 15.5 pt. Every time I open this file, the font is changed to non-boldfaced Calibri and the hanging indent at 2" is removed. The font sizes of 13 and 15.5 are preserved. The default font on this system when I created this file was 13 pt Calibri.

My Win 10 version is 10.0.19041, Build 19041, and my Word 365 version is 1908 (build 11929.21008).

Can anyone tell me why this happens and how to correct it? It's really annoying...

    Thank you.

I'm attaching the first page of this file. Interestingly, when I opened it on my personal computer running Win 10 and Word 2007, the first line formatting did not change from Arial to Calibri and the 2" hanging indent remained in place.
