"Windows 10 installation has failed" error

I'm trying to upgrade my Windows 10 but no matter what I try I keep getting an "Windows 10 installation has failed" error.
I've tried uninstalling the antivirus, using different DISM cleanup options, using the disk cleanup tool...

The CBS log contain lines such as:
    2017-04-22 12:24:45, Info CBS Failed opening file: C:\WINDOWS\Logs\CBS\CbsPersist_20170417185800.log [HRESULT = 0x80070002 - ERROR_FILE_NOT_FOUND]

    2017-04-22 12:24:45, Info CBS Failed compressing file C:\WINDOWS\Logs\CBS\CbsPersist_20170417185800.log [HRESULT = 0x80070002 - ERROR_FILE_NOT_FOUND]

    2017-04-22 12:24:47, Info CBS Failed archiving backup logs [HRESULT = 0x80070002 - ERROR_FILE_NOT_FOUND]

2017-04-22 12:25:04, Warning CBS Current tick count lower than last tick count. [HRESULT = 0x8007000d - ERROR_INVALID_DATA]

    2017-04-22 12:25:04, Error CBS Session: 30587730_2601731245 failed to perform Synchronous Cleanup operation [HRESULT = 0x80070002 - ERROR_FILE_NOT_FOUND]

    What can I do?

What media are you running the upgrade from? USB flash drive, DVD, mounted ISO file?
