Here are some things I'd try
    - after each change, restart the machine and try the upgrade
If any of these suggestions help, you don't have to continue with the other suggestions
... but please do post what helped

    1. Control Panel > Region > Administrative tab > Change system locale
Select English (United States) - yes even if it is already set
      Press the Copy Settings button
      Select both
Welcome screen and system accounts and New user accounts
      Press OK

Your boot might be corrupt, even if you can boot, the upgrade might be more finicky (buggy??)
Startup Repair - Run in Windows 10 - Windows 10 Forums

    2. Disable 3rd party AV pgms - enable Defender

    3. Restart your machine in case there are any system operations pending

Click here to download Old Timer-TFC.
      >> save the application to your Desktop.
Old Timer-TFC is a standalone application, there is no install.

Save your work and close all open windows.
TFC will close ALL open programs including your browser!

Right click, run as administrator TFC

Click the Start button to begin the cleaning up temporary files and folders.
Do not work on other things while TFC is running - most applications use some sort of temporary files. Just let TFC run by itself on the machine until it completes.

Restart your machine immediately after TFC completes.

    4. Perform a Clean boot
Clean Boot - Perform in Windows 10 to Troubleshoot Software Conflicts - Windows 10 Forums

    5. Check for previous upgrade folders (show hidden: $Windows.~BT, and $Windows.~WS) on your c:\ drive - clean them up
Disk Cleanup - Open and Use in Windows 10 - Windows 10 Forums
      I usually check everything

I'd also like to see your Disk Management.

If you've already downloaded the custom MMC, go to Step 2 to launch it and then grab the screen shot.

Step 1: Download this zip file (contains dmDskmgr-vd.mmc)

Step 2: Double click dmDskmgr-vd.zip to open the compressed folder
Double click dmDskmgr-vd.mmc to launch the custom Disk Management console
You'll get an output similar to this:

Press Alt+PrtScn to grab a snapshot of just the Disk Management window
Open Paint and Ctrl+V to paste it, then save the image
Attach the image to a new post.

Option 2: Use Disk Management in the x-Menu (right click the Windows logo, select Disk Management)
Select View > Bottom > Disk list
Maximize the window and grab the screen shot.

Good morning, Murdabenne! If you haven't already, try to get that setuperr.log for @simrick. And the CBS log. They may hold a lot of clues. See ya later.....

For CBS, unless you need more TC, I only want the stuff from this command:
    Code:
    findstr /c:"[SR]" %windir%\Logs\CBS\CBS.log >"%userprofile%\Desktop\sfcdetails.txt"

I was gonna post that, but didn't. I use that all the time, else you gotta wade thru 10K lines of Who-Cares!

Yeah, although I recall him saying that sfc came out okay - no integrity violations found?

    @Murdabenne
I have gone through this thread and made notes of everything, so it's fresh in my head now.
When you have time, please answer the following questions, and/or provide the information below:

This system was originally upgraded from W8.1Pro to W10Pro, but then W10 was clean installed once, right? Any specific problems for the clean install?
How many/what kind of HDDs are connected?
Please also list what is stored where. (i.e. where do the libraries point? Programs are installed on a drive other than the main OS SSD?)
    Disk Management screenshot?
    No Overclocking, right?
Defender is the only AV now and Windows Firewall.
Was MCT tried with “run as administrator”?
    Need Ccleaner installed programs list
Any Speccy or any HDD/System monitoring programs?
    Was CHKDSK run on SSD?
    Fast Startup is off?
Are you currently in a Clean Boot state?
    BIOS up-to-date?
    Any anti-telemetry programs used, ever?
Was startup repair tried, as suggested by Slartybart?
    Was OldTimer TFC tried?
Was this tried: Device Manager – right-click and update each manually-let Windows check automatically
Was this tried: Upgrade with internet, upgrade without internet

I would like to see your current setuperr.log.
    C:\$WINDOWS.~BT\Sources\Panther\setuperr.log
You can post it here, between CODE tags (use the # button).

I would also like to see the abbreviated results from CBS log. At an administrative command prompt, type
    Code:
    findstr /c:"[SR]" %windir%\Logs\CBS\CBS.log >"%userprofile%\Desktop\sfcdetails.txt"
and post here. (I realize the results were clean.) If your libraries are pointing elsewhere, you'll have to go searching for it - you know that, right?

Open Device Manager, select to show Hidden Devices, and post a screenshot, with storage volume section expanded, and anything else that might be unknown/unexpected. Better yet, just expand everything and post it that way.

I looked through your previous pastebin log, it showed, (among other things), problems with too many INF (drivers) trying to migrate, and with the BCD/creating recovery. Would like to verify those still exist. Then we can proceed further. Thanks.

    Last attempt. Failed. "Something Happened"

OK - I jumped thru all the hoops. Still get the stupidest error message Microsoft has ever written "Something Happened" .

The findstr produced an empty file - so I loaded the log into notepad++ and searched manually and nothing in there with [SR] or SR other than server entries that appear routine. FYI, This was a scratch install to a fresh 500gb EVO SSD, after the initial upgrade from 8.1 pro 64 bit to get the registration set. UEFI setup, I am running SLI NVidia graphics cards and dual monitor setup, but I've gone as far as pulling those out, uninstalling the NVidia drivers and going with the Intel built in stuff and only a single monitor. Still negative result. I even made sure the BIOS is flashed to the latest. The system is installed to C: and data and media are kept on D on a 1G 7200rpm WD drive, with my old SSD standing in at 256GB as drive F ("Fast") with a few things installed there for speed (mainly for loading, like games), with the DVD/BLuRay on E. I let windows set up all the partitions GPT. when I did the original install and have resisted the urge to tinker.

And it has gotten worse: the damnable update automation has greyed out my ability to change the reboot time/date and put it off. This infernal machine has restarted 4 times already today under control of Microsoft mindlessly bashing its head in against whatever error it has created. I cannot stop the reboots, so I am stuck with saving my work, waiting out out, reading yet another failed attempt in the Update screen, trying MCT method, watching it fail with "Something Happened". I'm ready to take a hammer to this thing, and send the remains to MS Headquarters.

I've had enough. I am now forced to throw away my day off Thursday to do a scratch reinstall unless I see something more here. Thanks for all the help. I now get to spend parts of my time off for the next week reinstalling applications, getting my applications re-downloaded, re-installed and re-registering them (and hoping I have all the serials and passwords in my gmail). If it weren't for specific apps and some games, I'd be tempted to format this thing and throw some version of Linux on it and never look back. I still have an i7 Mac mini, and I can use it for the time being to work on a research presentation for the cardiac group, but I really don't like using my Mac -- I think its overpriced and its such a walled garden limiting me, and I also don't flow my work the way they want me to. Despite all that I have to credit Apple, because their Mac Mini and OSX have NEVER bitten me in the ass this hard.

Microsoft and their useless tech support can rot in hell for putting me through this. All y'all here,on the other hand, have been what they should have been - patient, informative and sympathetic. Thank you again. I'm downloading a fresh copy of the latest 1511 ISO, and making a UEFI USB install setup, and backing everything off here one last time. I'll be reformatting after I get a nap in tomorrow morning.

For the historical record, here is the entire contents of the setuperr.log
