I found out that the Windows updates can have different numbers. I couldn't find KB2990214, but I found the one I needed. Unfortunately, it didn't uninstall the Upgrade prompt within Windows update. I re-installed Windows 8.1 and all is well.

As I stated in the subject line, Windows 10 setup, whether using the regular update method, running the ISO from within Windows, or using the Media Creation Tool, only asks for product key and there is no option (button) for 'Upgrade'.
