Thanks so much for your help simrick.
The device does not come up as unknown but as mass storage controller. Windows doesn't find a driver for it.
Looking on the W7 hard drive the device is O2 micro integrated MMC/SD controller and O2 micro integrated MSPRO/xD controller.
Just finished another upgrade install try but using the Tech Bench ISO on USB key on the original W7 HDD this time. Stuck again, same place. So little Sister will have to give me the go ahead if she wants me to upgrade to W10. Once she sees it on the other HDD she will have to have it...lol.

Oh yeah - I see, storage controller. You have got to find a driver for that or little sis will not be able to read any SD cards. Might want to ask on the Toshiba support forums and see if someone can identify the proper driver for you.

Here are some W10 drivers - not sure if they will work, but, make a system restore point on the W10 OS and see what happens.
    O2Micro Media Card Reader Driver for Windows 10 (32-bit, 64-bit), 8.1 (64-bit), 8 (64-bit), 7 (32-bit, 64-bit) - ThinkPad W540, W541 - Lenovo Support (US)
If it doesn't work, just remove it, or use system restore to go back.

Apparently that is what's preventing the laptop from doing the "upgrade".

One thought - have you tried the upgrade with the TechBench ISO, disconnected from the internet, instructing it to NOT check for updates during the upgrade?

I'll give the Tech Bench ISO another go tomorrow, without checking for updates. Do you think it would help to uninstall the two card reader devices and their software from device manager in W7 and then trying the W10 install again?
    Thanks for the driver links.

I would first try the drivers on the W10 OS HDD and see what happens. I would also post in the Toshiba forums - they are pretty good/fast to reply if they know the answer.

If you can get a driver to work, why not just migrate her data to the W10 install and be done?

p.s. On the W7 OS, try the upgrade without an internet connection first and see what happens. Make sure you have all drivers backed up before you go deleting everything. Hey - you've gone through this so many times, you are an expert now!

Windows 10 doesn't like the Lenovo driver but...
Wowzers, I was walking by the laptop with my camera and thought, you don't suppose it will read the SD card....LOL...IT DID. Seems Windows 10 is ok without the driver.

Oh cool. So you installed in compatibility mode?
Please, could you post the link to the "page", in case the driver download gets moved in the future by Toshiba, and then we have a broken link in the thread? Thanks!
