It is suggested that AV software can interfere with repair. I would assume that you disabled all onboard security beforehand?
If not then it could have been what caused the problem, How to fix? I don't know other than reinstalling windows.
Maybe a boot trace might help.

However the log will likely be huge and I'm no expert on diagnosis. There might be something obvious that shows up.
EDIT. It probably won't help as I can't see how it can log activity before the lock screen shows up.
