Exact same thing is happening to me. Did you ever resolve? MS icons are fine for me too. All 3rd party icons are fine, except for the start menu....
    still not fixed.

I'm on build 10547 and have this problem with several icons only having a blank place card.

Changing the actual filename and then going into the shortcut and changing the target file name to match the new changed file name the program graphic now appears in the blank place card.

It's like the file name is being blocked from sending the graphic to the place card.

    You can try:
    1. Disable all the third party Shell Extensions: Right Click on Desktop take long time to show up - Windows 10 Forums
    2. Win+R->Msconfig->Services. Put tick on "Hide all Microsoft services" and disable all
    3. Task Manager->Startup, right click and disable all startup items.

If you're on the fast ring install build 10565 as this fixed it for me, happy days

But the option of being able to copy a pinned document on say an Office application is now removed (prior to this build it was in the context menu but the copy just didn't work)
That's right MS, if you can't fix something then just remove it
Also, this build hasn't fixed the issue where the start menu delays opening and the search box doesn't respond (end Cortana task or restart Explorer task to fix)
This is all getting so tedious, fix one thing then break/remove another
Win 10 is slowly becoming less like an upgrade and more like a downgrade
MS, you could have saved yourself and everyone else time & money and just not bothered with Win 10
Just what is your agenda as I am baffled?

Any fix to this issue? I still have this problem on win 10 home following an upgrade from 8.1.

I have tried rebuilding the icon cache, thumbnail cache, SFC Scannow, remove and reinstall software, copying shortcuts from the different start menus (roaming vs all users)

    All to no effect.....

Ensure you're on the latest build 10586.14 as it may fix your issue

Just tried something different... I tried changing the shortcut icon for Adobe Bridge. In Bridge .exe there are two identical Icons, with the first icon selected I get the paper icon in the all apps menu, with the second icon selected I get the correct Bridge Icon!! could someone with more brains than me and adobe bridge have a look at these icons and tell me what the difference is.

I am starting to think that the start menu has some specific icon requirements that are not met by certain bits of software i.e menu requires 256x256 icon, where only icon available is 128x128, or something like that......Of course I may be talking complete rubbish......

Using resource hacker I have saved the .Ico for the non working bridge icon, then changed the shortcut icon so that rather than using the icon within the .exe it uses the standalone .ico, and it works perfectly. Now totally confused....

I don't think that's the issue because I've seen people post broken icons that do work on mine. Not to mention I have Windows 95 software where the icon works just fine.

There is just a bug in Windows 10 causing is, and the Microsoft of 2015 is all about making random changes instead of FIXING stuff.

Microsoft really needs to leave Windows 10 alone for six months and go on a fixing spree. No new features...just fixes.
