    The fact that each of these scenarios can be accomplished by an 'Undo' action would make it my prime suspect.

    I've experienced this behavior before myself, which is what prompted me to comment.

    I guess it happens every couple months or so. And I'll only really catch it when it's been more obvious, like noticing a file I've deleted that then mysteriously reappears on my desktop after I've closed all my open windows.

    For me it can be trace back to working in some editor that has me using Ctrl - Z a lot like Photoshop or email, where I'm distracted between open windows, and then neglect to return focus back to that program's window before my key presses, such that the Explorer process then sees it as an 'Undo'.

    I guess the question worth asking is how frequently is this happening?

    If it is happening a whole lot, it's possible it's a bum keyboard that's now sending the wrong keycode.

    And I apologize ahead of time for what might sound silly to ask but, I'm finding with my eyesight more challenged these days I'll often click on icons absentmindedly. Like the 'Undo' icon in the Quick Access Bar atop an Explorer session thinking it's a Back or Browser Refresh.

    Sometimes, if I catch myself soon enough afterwards, I can hover other the 'Redo' icon long enough for the tool tip to tell me what I'd undone by mistake.


    This tool tip is also helpful after I've accidentally moved an important folder, usually when I'm scrolling the bar between the navigation and detail panes while the left mouse button is down, only knowing I just moved something inadvertantly, but not really seeing what or where it's gone.

    think that is the case too. I use a lot of ctrl + z for redo and even sometimes ctrl + x for cut. but sometimes i'd press wrong key due to right beside each other. havent come across this now so and Ive been more careful on the key press, prob was the cause.
