Please download GPU-Z and post screenshots of every tab.

Tried to install and run it, but it said something about not being able to run in safe mode. Then it said "Could not delete driver: the specified service has been marked for deletion". Should I uninstall my graphics drivers so I can get out of safe mode?

Uninstall drivers in safe mode with DDU, then download this driver and install.

Sorry, but can you try that graphic card in another computer for test?

Given the number of errors in Reliability History, probably the only effective method would be to Clean Install. If it is possible to run sfc and dism in the present setup, then give it a try.Not being able to install drivers points to a corrupted system.

No, but I can borrow a spare graphics card off a friend, would it be worth trying it in my PC to see how it runs compared to my GPU?

I can't get any of that to work in safe mode. Do you know any way around this? It's so frustrating that I can't try anything outside of safe mode without getting a black screen.

But thanks for all the help so far everyone!

Put in my friend's old GPU (Radeon HD 6750) and it booted up perfectly; no weird lines, no black screens, nothing. Is it safe to say my GPU is screwed?

I think your GPU is dying, try put it again and see how it works, but i think you need to think about buying a new one.
    Cheers

Just one another thing you can try, flash different version VBIOS on that graphic card.
