Yes, that's what I was thinking anyway.
As I said before, thanks to the Media Creation Tool, I have W10 1903 on USB. It will not be just yet - it will have to be a quiet time like very late tonight, or very early tomorrow morning as it will take three and a half hours on this PC, I've done it before. That includes searching for and installing updates after it has booted back in to the Desktop/Start etc.
Anyway, thank you very much for your patience and assistance so far, you've really done your best with me.

Well, in the end I went for it - and a Windows In-Place Repair Upgrade was the cure for it; as with so many things. So now everythings back to normal. It took some time, especially given that W10 1903 was on a USB memory stick, and only USB 2.0 ports on this PC, but it's done for now.
I'd just like to thank everyone who replied, especially Dalchina who gave up so much of their time yesterday for me.

You probably know that if the problem is specific to a user account it's very unlikely an in-place upgrade repair can fix that. Whilst some causes of this symptom can be easily attributable to changes a user can make (e.g. via Indexing Options), others may not be, and there's no easy way to find a specific fix.
Well, I don't quite know why anyway, but I've found that works in the past when all else has failed.
