Ok, try uninstalling this update: it's caused various problems to some users.

Bear in mind we're playing guessing games- what's different between 1809 and 1903 for you.- there could be other driver differences that matter, for example.
What sort of mouse are you using? If wireless, please try a wired mouse.
Is your PC a laptop or desktop? Still no model number..

i use build 1903 and i have did clean boot how have say dalchina user and the problem persist always, i doubt will be bug windows search and clicking bug menu

i have uninstall KB4515384 and i will be back 18362.329 and the problem persist. i use desktop pc
Sorry we can't provide an exact solution- this is not a familiar issue and seems Win 10 build-related.
Noted:
You've done a clean install and a clean boot as well.in safe mode doesn't give me any problems.
Can you confirm that after the clean install you saw this problem before installing any other programs- including security software?
Now, you've also tried different graphics drivers, and that hasn't helped.
I assume you see this happen when your mouse pointer is over the menu in question.
Have you tried a different mouse?
Returning to this:
If it's not mouse or graphics driver related, then the following remain as differences:in safe mode doesn't give me any problems.
a. In Safe Mode different versions of some drivers are used (e.g. graphics driver, mouse driver).
b. Many things are not running- including some services and drivers.
We also know it does not happen with build 1809.
I see no simple solution.
One possible - but lengthy- approach to try to identify a cause- would be to use Autoruns (free from MS).
a. In Safe Mode save the list of all startups
b. In Normal Mode, progressively disable startups to match.
At some point, there's a chance the symptom will stop.
No guarantees, and it stll leaves the question as to what you might then do.
the problem is right click not working when i want see menu : run, open with administrator,open location file, add to start.

okay resolved the problem was xbox one controller not working on usb . i have change new controller xbox one model play. now menu not goes.
