The same apples to a clean install if your machine is set up to boot from the USB before the HDD. If you leave the USB in the first time it restarts you'll be wondering why you are back at the beginning of the install.

Tested 1903 on a few spare loaner laptops that I had around the shop. Zero issues so far. Plan on making some images soon for deployment.

Yeah I can see how that would be a bit confusing!! lol I must have been lucky. Ive had PC's... both laptops and desktops for years. Ive never had one that was set up to boot up from any other drive other than the hard disc as the first boot option. Ive had Packard Bell, Compaq, Acer and for the last few years Ive always bought Dell. Ive always just tapped F12 during bootup and selected the USB flash drive from the one time boot menu and never had any issues

The part of 1903 that worries me most is this:

Inappropriate drive reassignment can occur on eligible computers that have an external USB device or SD memory card attached during the installation of the May 2019 update.

Note

The drive reassignment is not limited to removable drives. Internal hard drives can also be affected.

All subsequent upgrades I have done manually by using the media created by the MCT. I prefer to make the ISO rather than the USB then mount the ISO and run Setup from the virtual DVD. I do this partly because it avoids having to remember to remove the USB at first reboot, but also because it is easy to make a USB from the ISO should I need one, so the ISO is the best of both worlds.

My system uses one internal SATA drive for the OS, and another for data of various kinds. I keep consistent backups of both drives, but I usually assume the data drive to be safe from install glitches as it is not used for the OS. Has anyone installing/using 1903 seen any modifications to non-OS internal drives, as mentioned in the above-referenced notice from MS?

My Samsung (system two below) has no such 'one-time boot' menu. The only way to get that to boot from a DVD or USB is to go into the bios and change the boot order.

It's a pain to have to go into the bios every time I need to boot a usb, so I have it permanently set for CD/DVD first, USB second and only if neither are present will it boot from its HDD.

That's a new on me. I thought all PC's were equipped with the one time boot menu...obviously not. When clean installing then you have no choice other than to remove the flash drive or DVD before it reboots during Windows setup or you would just be in a constant loop and the installation would never complete. Sounds a bit awkward but I suppose its what each of us gets used to by our own individual experiences with PC's
