I can't seem to replicate it at will. It just does it when it wants to. Right now, I don't have any apps doing it.

Disable anti aliasing in the NVIDIA Control Panel. (Try Global Settings or set it for each individual app).

If I disable it, will the in-game settings for each game still enable it, for that game? Or do I have to disable it for each affected app?

Yes, games most likely will get affected.

This issue you are facing is mostly because you are using an insider version of Windows.
Unless necessary, I would suggest you to switch to a release candidate version of Windows.

I've been having the issue regardless of whether it's an RC build or insider. Still out of focus.

Also, disabled AA for Discord, and it's still going out of focus.

    Hmmmmmmm, strange.
Can I know a list of apps this is happening with and also which graphic card you are using?

I've seen it in various apps including NVIDIA GeForce Experience, Discord, and WeMod. I'm using an NVIDIA GeForce 1060 GTX.
