Is this perhaps occurring in Edge? There's a read aloud option there, independent of Narrator.
I wonder if Chrome has its own..

I am using chrome, so I don't know about Edge..
I did a quick google search and it seems like you need to install an addon in chrome..

    unless someone knows otherwise

You could try an experiment just to confirm it is the Narrator - change the voice it uses and see if it matches what you hear.

Okay I think I have a working theory of what is causing this problem..

Just had an insight into the problem!

I think this problem might be related to https://discordapp.com/

I got this Chrome tab opened permanently since october..and it has chats going on in the channels

And I just checked in the settings on the site
    and there is this setting..

    Text to speech

Allow playback and usage of /tts command

I think this might be the reason why there is random narrator voice talking crap
and yet there is no narrator software showing in the task bar.

I think it starts talking when someone sends this /tts to the channel.

I am pretty certain this is the cause.
I will test to see if this is the case.

Did you turn the narrator off in the "Ease of Access Center" from the Control Panel? You said you made sure "Use Narrator" is turned off, but were unclear as to how you accessed it.

Just an update on this problem

Okay I now know the cause of this annoying problem.

It is definitely the discordapp website.
I was just looking at a channel, and it read the message on that channel with the same robot voice.

Well, at least you got confirmation that it was something that you are not responsible for. I was partially on the right track when I stated that it was possibly something that was changed recently; although I'm guessing not by you.

Like you stated, half the battle is knowing what is causing it, and the other half is figuring out how to control or stop a website from overriding local settings on the computer.

I would start looking for workarounds that are proven to work in your scenario that don't interfere with how local settings function on a day to day basis.
