You're looking at 2 different things.
The second- the SMART parameters essentially the state of the physical disk.
The first relates to the integrity of the file system on that disk.
The first probably relates to the state of the so-called 'dirty' bit.

- which scan? some version of chkdsk? or something else?When I run the scan it says there is no problem!
OK, I ran the fsutil dirty query d:
and it returned Volume - d: is Dirty
But why?
The problem usually happens when I have not accessed the computer of the disk for a while. Since this is an external USB disk, I wonder if it takes to long to start and that's why the problem occurs.

I note the drive is formatted exFAT. False negatives in Win 10 are common with any drive other than NTFS, particularly exFAT and FAT32 (even though there is never anything wrong).
Backup the contents, reformat the drive NTFS, return the data and you won't see that annoying message again.
Additional note : This is probably how it came out of the box. I always practise that every new USB device - SSD, HDD or flash - is not just re-formatted but has all partitions deleted and new NTFS partition(s) created (unless FAT32 is required by the device it's to be used on ... then I just live with Win 10 foibles about not playing nicely)
I have deleted the partition and created a new one with NTFS
So far I haven't had any problem!
