    Why is Windows throwing bogus USB errors?


    I have a freshly formatted Optiplex 9010 with 21H1 and it keeps telling me some non-existent USB device has malfunctioned. This occurs even with the USB keyboard/mouse disconnected! I found some entries in the device manager and disabled them. Still every so often, Windows complains. What might be causing it? The system has the latest BIOS, too. Is there a definitive way to turn off the notifications?

    It's likely that you BIOS/Firmware needs to be updated. If that link doesn't help you download Dell Update from dells support website using the look up tool for your particular service code or service tag and run the Dell update software to see if it finds an update for your firmware or bios. Please note that updating files and firmware can be a dangerous process if you lose power during the update you might brick your system

    As I indicated above, the BIOS is already the latest. It also has the latest chipset drivers.

    Did you try the chipset drivers using the intel driver assistant? If you have no USB devices when it throws the error and your drivers are all up to date it looks like driver corruption issue. You can try repairing the system using the sfc /scannow DISM restore health method. If it's a clean OS the storage device might be the problem I would run a check on the disk itself using chkdsk /f or /r or run a smart check on the disk. Sorry about missing that you already check the BIOS. I missed that somehow.

    It's not driver corruption. I installed Windows fresh twice and the behavior is the same. I learned of the program USBDeview and this program can truly disable USB devices, which Device Manager, for whatever reason, can’t do. And I disabled them. They still appear enabled in Device Manager, but they are actually disabled and the messages stopped.

    Try to install some optional driver updates as from Settings:

    Nowadays, when I clean install my system, there always are entries in Device Manager with yellow exclamation mark. Perhaps, it's not like in your case but you can give it a shot. So, after installing the driver updates, in my case, all those marks disappear and the Device Manager looks nice and clean.
    Sorry but I can't come up with something better.

    This won't be solved with drivers. There's something odd with the hardware. Only disabling the affected device entries with USBDeview is the solution.

    Does the bogus device show in Control Panel>Devices & Printers ?
    If so,then right click the icon and try ---remove device---troubleshoot---properties-- - and see if they get rid of it.

    You can’t remove them by removing them from the Device Manager. The devices are still "connected" so they return on reboot.

    The suggestion only considered CONTROL PANEL.
    Nowhere did I mention Device Manager.
