Yeah I've run "chkdsk C: /f /r /x" 3 times yesterday, during bootup, nothing found. Only thing I have not done is run it from a USB drive .....also this error is reported on ALL My drives, not just C: drive.
Last image update is from 9 days ago, right before our last updates. Was thinking of trying restoring back to that date. But I don't have any other issues .....

If it is affecting all your drives it could be either a controller issue or a driver problem.

Funky driver? Uninstall the SATA drivers and reboot? Re-install them? I know, I'm grasping at soggy straws!
Interesting idea, could be a driver issue, as after I did the clean install 2 weeks ago, Windows did install its own drivers, and they looked different then my original drivers... BUT didn't have this issue until just recently, last few days.
