Hey TC - have any idea how I fix this? I've already done app reset, sfc, dism, WU reset (wouldn't update either), and even did an in-place upgrade - still there!

Powershell - then see if you can re-install from store.

    Get-AppxPackage *windowsalarms* | Remove-AppxPackage

Then try to re-install from store:

I’m thinking the problem may lie in syncing via your MS account. Since W8/8.1 > OS Settings, Store app library, Favorites, etc. are backed up within a hidden folder within your/one's OneDrive.

Either try turning off syncing on one OS or ideally use two separate MS accounts.

Not sure what the external HD problem is, though.
I am back in that build now and all is well, I had to remove a program to get the external drive to work again. Thanks

Thanks TC. That got rid of the entry in Start.
I still have some things listed in Start that are almost grayed out, but the links work, so I am stumped on this one.

    Just seems to be random.

I have a Visitor Account setup on this one - wonder if that is part of the problem.

Restricted Visitor account? Try upping it's privileges to Admin and see if the "gray out" goes away. Just a thought.....
