OK, so "startup repair" from recovery mode didn't solve the issue, have you tried "system restore" in recovery mode?

OK, let's try to run chkdsk on the disk.
in recovery mode open up command prompt and type:
    Code:
    chkdsk /R C:
press enter and wait for command to complete, then report the results and try booting into safe mode, and system restore again if safe mode fails.

this assumes Windows is installed in C:
you can use diskpart in command prompt to check out volume letters first:
    Code:
    diskpart
    lis vol

I've run chkdsk three times already, there was a lot of minor errors first time, now it shows 0 errors every time.

type this command into command prompt this time:

    Code:
    Dism.exe /Online /Cleanup-Image /Restorehealth
reboot and next time again into cmd type:
    Code:
    SFC /SCANNOW
again reboot and rerun system restore.
and report results of above commands.

I think I need to specify image? Cause "online" probably refers to recovery mode on X:

    ah sorry try this:
    Code:
    dism.exe /Image:C:\ /Cleanup-Image /Restorehealth
if SFC /SCANNOW doesn't work as well try:
    Code:
    SFC /SCANNOW /OFFBOOTDIR=C:\ /OFFWINDIR=C:\Windows

So, I disabled recovery mode with bcdedit, and now it shows this:
    Attachment 209749
Edit: I replaced this file with one from healthy pc, and now it's all back to stage 1 - 0xc000021a and no fix working.
