Can you try the Ctrl-J Clear downloads item method then?

Refresh Firefox.
"This file contains a virus or malware." download popup won't go away | Firefox Support Forum | Mozilla Support
First make a list of extensions so that you can reinstall them.
Refresh Firefox - reset add-ons and settings | Firefox Help
If I had the problem you are having, I would just uninstall, then reinstall Firefox.
Use REVO Uninstaller (free) to uninstall Firefox and do the "scan for leftovers", and remove whatever REVO finds.
Then... search the Windows drive for Mozilla and again for Firefox, and remove whatever it finds... EXCEPT for things that are in the WinSxS folder. Do NOT remove any Firefox or Mozilla entries in the WinSxS folder.
Then reboot and reinstall Firefox.
As for extensions... it could be that one of the extensions you're using is causing the problem.
In Firefox you can choose: Help > Troubleshoot Mode... which will start Firefox with NO extensions running.
See if the problem still exists.
