If you actually wanted to restore, yes, you would insert the other pen drive.
But you can check the menus without doing that.
Imagine 2 situations:
1: Your hard drive still works fine, but Windows is seriously fouled up and you want to restore.
You could choose "Macrium" from that menu you mentioned yesterday instead of "Windows". You'd end up at the Macrium interface ON YOUR HARD DRIVE and then go through the restore process. You wouldn't need to use the rescue USB stick because your hard drive is still working OK and it's just Windows that is haywire.
2: Your hard drive is totally dead.
You'd presumably go down to the repair shop WITH BOTH OF YOUR PEN DRIVES. You'd tell the guy you need a new hard drive and that you want him to install it. He'd then use your pen drives to restore your image to the new hard drive.
If you are qualified to replace the hard drive in the laptop yourself, you could do it all without going to the repair shop.

The hard drives in this laptop are both M. 2 drives i installed the second one my self which has all my workstuff on it and is backed up to two external drives. i use Goodsync for that but Goodsync wont let up back up the OS disks. How often would i need to update the rescue pendrive and the other pendrive that has the image on it?

As said don't use a pen drive for system images
Use a real hard drive make a folder name and date it point reflect to it
Do it as many times as you can weekly/ monthly.
Recovery media well that usually needs to be updates after large build updates.
Also remember you can create system images in the winpe recovery environment too.

The image pendrive: I do it monthly. You can do whatever you like, but the rule of thumb is to make a new image more often IF your system is constantly undergoing big changes--and less if it isn't. Somebody who is doing a lot of testing and experimentation might make a new image weekly or even daily. Somebody who rarely installs new applications and just uses the PC for email and shopping might make a new image once or twice a year.
The rescue pendrive: I think Macrium may mention in the release notes of new Macrium versions when you need to update the rescue disk. But I'd probably re-do it once or twice a year anyway. You don't need to do it for EVERY new version of Macrium. You should periodically try to boot from the rescue drive anyway---just as a test. If it won't lead you to the Macrium interface, it is TOTALLY useless.

I use the winpe environment exclusively failure rate is near zero best way to know if the recovery media works too
If it doesn't you'll know it needs to be updated
Only trick is to make a folder for the system image in advance for some reason you can not create a folder for a system image there.

I need coffee, if the below has already been covered in earlier posts, ignore my post.
If that pendrive boots and brings you to a macrium menu for backup/restore, you have succeeded. Now, use two twin pendrives, do a backup onto pendrive A, then do it again onto pendrive B. Use A for most restores. If A ever fails to backup or restore, if B contents = A contents, then use B. However, I vote with others: best to use external USB HDDs.
