You can run MR when logged in, then create a new base image after upgrading to 1903.
As for the 'rescue usb stick' as your thread title refers to MR, by this do you mean MR's bootable medium?
I have created a new image of upgrade 1903 using MR. By 'rescue usb stick' I indeed meant MR's bootable rescue media created on a USB stick. I have such a stick from when I backed up the 1803 version, so my question really is this stick compatible with the 1903 version or do I have to create a new one?
There should be no particular reason not to use it as far as I know. As your PC boots from it, should be fine.

Easiest option is just to update it anyway, although there should be no need to do so.
I always update the rescue USB stick after MR is updated or there is a Windows feature update since it's quick to do. Consider adding a Reflect boot menu rescue option also.
The Macrium rescue media does not care what version of Windows (if any) is in the image being restored. Once you have booted from the Macrium usb stick you can restore anything. I have successfully restored Macrium images of XP and W7 machines with Macrium rescue media made by a Windows 10 machine.
Ideally you should create a new Macrium rescue stick whenever you upgrade your installed copy of Macrium. But as long as it is for the same major version of Macrium (currently 7.2) an older one should still work without problems.
