No. It adds to the files and folders already there. Took me by surprise the first time I tried it, I was used to making Win10 recovery drives where the USB is wiped before creation.
There are prerequisites. The USB has to already have an mbr partition table (some have none as formatted by the manufacturer) and be Fat32 formatted. If the usb isn't suitably formatted Macrium tells you to use Diskpart to make it so before continuing (which would clean the USB). There must be enough free space for the rescue media files.
Obviously, existing file(s) with the same names as the ones Macrium wants to copy to the USB will be overwritten, but all the rest will be left untouched.
Would it not been better if it had. Wiped it I mean.
I always format the USB drive before creating a new MR media to be safe. I even format the MCT to be safe before downloading the new RS build.
Do you need to make a new restore media for each image backup?
No. Older restore media should be able to restore images made by the latest version of Macrium. Obviously you'd need to make new media if there were to be a major upgrade to Macrium (say, if v7 is ever upgraded to a v8) but minor upgrades do not require new media. It's always a good idea to use the latest restore media, but not strictly essential, as I showed here....
