I have a batch script that will create random generated passwords.
Now this is way past what I would ever need, but I've been thinking about this. (Oh no!)
The following is just where I'm starting from. A leaping off point.
Let's use your bank's website for an example.
There would have to be a program that you and the website use, and a code (mathematical?) for all the possible keyboard keys.
It might work something like this...
The program might create a hash of you IP?, and then use say one of 10 formulas, randomly, to change say, the letter A into some "other" key board, key.
Then, when you create the account at the bank, your copy of the program sends whatever pattern IT used to code your key presses, to the bank's copy of the program. This way the bank can decode, your password.
Each time you go to log in to your bank account, the two programs could exchange, whatever they used to scramble your password. It could be one of the 10 formulas, whatever the hash of your current IP is, etc.
It would still be crackable, but would take so long, that it wouldn't be worth it for most hackers of their hackees.
Ofc, and infection would still mess it up... but that would be true of w/e method you used.
