If I create a rule (in and out) to enable a program do I need to enable also ports that the program needs?

Windows Firewall will enable well-known ports associated with specific applications (programs). But if they want to use non-standard ports (e.g. 8080 is common for HTTP, which normally uses port 80) those must be added separately. That said, it can't hurt to enable ports you know an app will use as long as such usage has other means to prevent unwanted or illicit use (logins, 2FA, and so forth).

By default everything is allowed for it, unless you want to limit it further, then you can add port, IP, etc.
