Well I'm running Windows 10 Pro x64 (non insider) 1909 build 18363.720, anti malware 4.18.2001.10, anti virus 1.311.1095.0, anti spyware 1.311.1095.0, engine 1.1.16800.2, all default settings for Defender and firewall, no 3rd party network/ anti-virus/ anti-spyware etc.
Quick scan completes with no issue, full scan completes with no issue.
Latest Windows updates installed, KB4540673 on 10/3/20 and KB4551762 on 13/3/20.
No issues with OS or Defender.

It appears to be affecting only some users after the 4.18.2003.4-0 Platform update. And for some of them, only one machine but not the other.
For me it was both that choked up this message in the early AM of Tuesday the 10th, 8 hours before .719 was released but right after the Defender 4.18.2003.4-0 Platform update.
This 'Item Exclusion' issue still present after .719, .720 and after Wednesday's re-issue of 4.18.2003.5-0
When I re-imaged back to the day before Tuesday, when it was still running 4.18.2001.10 like you - no problem. Then updated the platform and the problem returned.
The fact mentioned by others, that Window's Catalog hasn't posted either 4.18.2003.4-0 or 4.18.2003.5-0 Platforms yet, only 4.18.2001.10, would suggest there's perhaps some problem.
Windows Defender Platform just updated again from:
4.18.2003.5-0
to
4.18.2003.6-0
Sadly this Item Exclusion Issue persists. ;o(
Clearly something's broken, and for enough people that they're flinging new platforms at me, as a guinea pig, to see if they'll stick.

Still on 4.18.2001.10 on all my systems here last checked, nothing new in Windows update either, maybe im lucky for once lol
4.18.2003.6 here on my home PC. 4.18.2001.10 on all my other pcs.
